TL;DR

FilterToolCallsMiddleware in @ag-ui/client keeps its blocked tool call id set on the middleware instance instead of per run. A stalled or interrupted run leaves ids on that shared set, so a later run on the same middleware instance can have unrelated tool call events wrongly filtered, or the set can grow without bound across many runs. The fix moves the set into a per subscription scope and clears it on RUN_STARTED so state no longer leaks between runs.

Who does this affect?

You are affected if you are using a version that falls within the vulnerable range and you reuse a single FilterToolCallsMiddleware instance across multiple agent runs.
